Your privacy is very important and I am dedicated to protecting your information.

This policy details how I, Nadia Quarcoopome, and MindKind Central obtain, store and protect your information. It also describes your legal rights and who to contact if you have any enquiries, concerns or complaints.

You and your information are protected by law and I can only use your information for reasonable and justifiable purposes. Under the UK General Data Protection Regulation (UK GDPR), the lawful bases I rely on for processing this information are;

I collect and process the following information;

​

  • Contact details including name, address, phone number and email address

  • Emergency contact details including name, phone number and relationship to you, the client

  • Personal and family medical information, including medical conditions and medications in use where appropriate

  • Occupation where appropriate

The personal information I process is provided to me directly by you for one of the following reasons;

  • To allow me to make contact with you in the manner agreed between us. This will include using your contact details to send session reminders via email and/or text message, to complete agreed tasks relating to sessions, to send secure session invoices, receipts and to arrange or rearrange sessions as needed. I will also use your contact details to send recordings of your hypnotherapy programme if you have agreed to this.

  • To respond to enquiries you may send to me relating to hypnotherapy, life coaching and anything related to MindKind Central.

  • Emergency contact information is provided by you to ensure there is somebody of your choosing I can contact in case of an emergency.

  • I ask for medical information to ensure I can provide the required service to you and ensure that it is safe and appropriate for you.

  • I may ask you for your occupation if it relates directly to your reason for seeking the services of MindKind Central. This is to allow me to understand your situation to the best of my ability and allow me to work with you as accurately as possible.

The information you give me is confidential, however, in the event of potential harm coming to yourself or others, or if required by law, I would be obligated to share information with the appropriate authorities.

As a responsible hypnotherapist I undertake regular supervision to ensure that I am providing the best service that I can. In these instances I may disclose certain session details in the process of my own development but, if the above exceptions do not apply, no identifyable information of any kind would be disclosed.

How I store your personal information

Your information, for example forms and contracts, is securely stored in a safe location in a locked filing cabinet to which I have the only key, and any session notes are kept locked separately and coded for added security. Any information received online is password protected and only I have access.

I keep personal information and session details securely for seven years from the end of the last session. I will then destroy all paper based information and fully delete all online documents.

Under data protection law, you have rights including:

Your right of access - You have the right to ask me for copies of your personal information.

Your right to rectification - You have the right to ask me to rectify personal information you think is inaccurate. You also have the right to ask me to complete information you think is incomplete.

Your right to erasure - You have the right to ask me to erase your personal information in certain circumstances.

​

Your right to restriction of processing - You have the right to ask me to restrict the processing of your personal information in certain circumstances.

​

Your right to object to processing - You have the right to object to the processing of your personal information in certain circumstances.

​

Your right to data portability - You have the right to ask that we transfer the personal information you gave me to another organisation, or to you, in certain circumstances.

You are not required to pay any charge for exercising your rights. If you make a request, I have one month to respond to you.
